Leadership Collaborative for Congestive Heart Failure: November 2002

Worked with the representatives of the Leadership Collaborative for Congestive Heart Failure to create a draft statement of purpose for implementing chronic disease management within the Province of BC. We also helped define the structure and format of the facilitated sessions for a November 1, 2002 meeting – Phase 1 of the Leadership Collaborative for Congestive Heart Failure. Over 80 health care professionals from around the province participated. We provided a facilitation team to lead small group sessions that elicited comments and encouraged buy-in to the Chronic Disease Management statement of purpose.

Drawing on the experience and perspective of health professionals at the provincial, health authority and provider level, participants provided hundreds of concrete suggestions for implementing Chronic Disease Management for Congested Heart Failure. These were incorporated into a document of potential action plans for implementation throughout the province.

Strategic Planning with the Medical & Pharmaceutical Services Division : September, 2002

Facilitated strategic planning sessions using Creative Planning Dialogues for defining a statement of purpose and goals for the Policy and Program Management Group of BC’s Ministry of Health that will guide the division’s strategic direction.
